PROMO MECHANICS:

I. Qualifications

  1. The Promo is open to all China Bank Credit Cardholders ("Cardholders") whose accounts are active, current, and in good­­ credit standing*.
  2. The Promo is open to new and returning customers of Medgate.

II. Program Details

  1. The Promo is available from February 1, 2021 to January 31, 2022.
  2. Cardholders are entitled to the following:

III. How to Avail of the Offer

  1. Visit http://www.medgate.ph/shop/promo/chinabank.
  2. Choose a Telemedicine Plan and select “Add to Cart”.
  3. Fill out the Billing Information and declare the number of dependents, if applicable.
  4. Proceed to payment and input your China Bank Credit Card details to complete the transaction.
  5. Wait for the confirmation email containing the unique Access Key and Activation Code within two banking days.
  6. Enroll the Access Key and Activation Code on the link provided in the confirmation email.
  7. Start using Medgate’s 24/7 Telemedicine services.

IV. Terms & Conditions

  1. To qualify for the promo, the Cardholder must use his/her China Bank Credit Card for payment at Medgate.
  2. The promo offer may be availed by the Cardholder with the use of his/her China Bank Credit Card but the Plan may be assigned to a different qualified individual.
  3. The promo offer may be availed multiple times for the duration of the promo.
  4. The promo is available nationwide.
  5. The offer cannot be exchanged for cash, credit, or other products.
  6. The promo is non-cumulative and cannot be used in conjunction with any other discount, promotions, and discounted items unless specified. In the purchase of goods and services which are on promotional discount, the senior citizen can avail of either the promotional discount or the discount provided under the Expanded Senior Citizens Act of 2010, whichever is higher. To avail of the latter, the senior citizen must call Medgate at +632-87050700.
  7. China Bank reserves the right to charge the discount to the cardholder’s account should:
    1. The transaction of the Cardholder be found to be fraudulent.
    2. The Cardholder violate any of the promo terms and conditions.
  8. China Bank’s Terms and Conditions (https://www.chinabank.ph/cbcctc/) shall govern the use of China Bank Credit Card.
  9. The Cardholder’s use of the Medgate shop shall be governed by the Shop Terms and Conditions of Medgate (https://medgate.ph/shop/termsconditionsshop) (“Use”), while the Cardholder’s use of the services of Medgate and its accredited doctors (“Services”) shall be governed by the Service Terms and Conditions of Medgate (https://medgate.ph/termsconditions) respectively. Any complaint or dispute related to the Use and/or Services shall be referred solely to Medgate. China Bank shall not be liable or responsible for any claim or loss suffered or incurred by the Cardholder due or attributable to the Services, Medgate and its accredited doctors. 
  10. In case of disputes related to the promo, the decision of China Bank with the concurrence of DTI shall be considered final.
  11. By joining this promo, the Cardholder confirms that he/she has read, understood, and agrees to be bound by the mechanics of the promo.

*Cardholders in good credit standing are those whose accounts are active and current and not blocked, delinquent or past due, not under any collections repayment program or special installment, non-investigation due to suspected fraudulent activities, or those whose Credit Cards are not reported lost or stolen, and those who have not otherwise, violated any of the Terms and Conditions governing China Bank Credit Cards.
